Impotency - Raising the issue
Not able to perform? Being shy about your erectile dysfunction problem may be a fatal mistake as it could be the first signs of a cardiac rrest, writes Dr WONG TECK WEE.
ERECTILE dysfunction (ED) is defined as the persistent (lasting at least six months) inability to attain and maintain erection sufficient to permit satisfactory sexual perfor mance.
Penile erection depends on two main mechanisms: reflex erection or psychogenic erection.
The two major causes of ED are organic and psychogenic.
Organic causes include vascular diseases, neurological disorders (spinal problems), hormonal abnormalities, genital deformity and medication effects.
Although ED is not a lifethreatening disorder, it has a profound impact on the quality of life.
It is a warning sign of silent/occult vascular disease that lead to the notion that a man with ED and no cardiac symptoms is a cardiac (or vascular) patient until proven otherwise.
Risk factors for ED (hypertension, diabetes, smoking, and hypercholesterolemia) are also risk factors for coronary artery disease (CAD).
Medications may affect sexual function in many ways, such as induced impotence, diminished libido, inorgasmia, priaprism, premature ejaculation, gynaecomastia and decreased vaginal lubr ication.
The medications include hypertensive medicines such as beta blockers, diuretics (hydrochlorothiaside and spironolactone), tranquilisers, antidepressants and digoxin.
ED can be caused or aggravated by some medications but altering drug regimes is unlikely to reverse the problem unless there is a definite temporal relationship.
Other possible causes should be investigated and treated.
The treatment options for ED include oral medications, vacuum constriction devices, selfinjection therapy with alprostadil, papaverine, intraurethral alprostadil suppository, penile prosthesis and vascular surgery.
All current available treatments are suitable for a cardiovascular patient and, if used according to the instructions, do not increase the cardiovascular risk.
Oral medications from the phosphodiesterase type 5 enzyme inhibitors (PDE-5) family, such as sildenafil (Viagra, Pfizer), tadalafil (Cialis, Eli Lilly) and vardenafil (Levitra, Bayer) have revolutionised the treatment of ED since 1998.
Despite the lack of head-tohead comparative studies, all three PDE5 inhibitors appear to have equivalent efficacy in the treatment of ED.
Sildenafil has additional efficacy data in the management of ED associated with spinal cord injury and antidepressant medications.
Tadalafil has the longest duration of action (up to 36 hours); this feature can be both beneficial (greater sexual spontaneity) and possibly detrimental (delayed adverse events).
However, the concomitant use of nitrates with PDE5 inhibitors may be hazardous and life threatening.
They must be avoided as they can produce significant hypotension and are potentially fatal.
No nitrates should be used within 24 hours of PDE5 use.
Patients who have taken PDE5 inhibitors and then taken to the Accident and Emergency section of a hospital with chest pain must inform the physician because the first line treatment for angina pain is nitrates.
Most men with ED do not seek medical advice, mainly due to embarrassment and social stigma.
They often seek unorthodox, offlabel and over-the-counter methods of treatment.
Self-medication for ED is dangerous because of the strong association between ED and CAD.
Recent toxicology study in Hong Kong showed a prevalence of PDE5 inhibitors and their structurally modified analogues in over-the-counter ED health products found in convenience stores and pharmacies. This is a market not too different from Malaysia with high penetration of herbal products. Herbal products purporting to enhance male sexual function may contain concealed substances.
The study indicated the extent of adulterations by drug analogues was alarmingly high.
Such analogues are difficult to detect by ordinary laboratory methods, and might be used in an attempt to evade regulatory inspection.
Without going through the stringent drug testing process, the adverse effects of these chemicals remain unknown and unpredictable in unselected general population.
Expressions of sex can range from the pleasure of being near someone, to kissing, to touching, to embracing, to genitalia stimulation and intercourse.
Physiological response of sexual intercourse is divided by the amount of physical activity and sexual arousal.
Sexual arousal response can be divided into four phases —excitement, plateau, orgasm, and resolution.
The cardiovascular response to sexual intercourse is similar to mild-to-moderate effort encountered in daily activities, such as moping the floor.
In fact, sexual intercourse is equivalent to extreme emotional response, such as anger, in triggering a heart attack. In most patients the frequency of anger is higher than sexual intercourse.
Sexual activity, combined with organic heart diseases, poses a certain, albeit low, risk of sudden death. Epidemiological, retrospective autopsies study in Germany showed less than 0.18 per cent (48 out of 26,901 autopsies) of natural deaths occur during sexual activity.
Extramarital sex may be more stressful because the partner is usually younger, and sex occurs more often following excessive drinking or eating. Often, there’s an unexpressed need to perform well with a new partner and the patient may be too embarrassed to complain about chest pain.
It also usually takes place in unfamiliar surroundings and the fear of being caught increases stress/ heightens excitement.
The risk of ischaemia during sexual intercourse with a familiar partner, in familiar settings, is quite low.
Angina pain, breathlessness, palpitation and giddiness that occur during or after intercourse are warning signs that should be reported to the doctor.
